Overview

The Retention in Care Specialist will conduct outreach to patients who have not attended an HIV medical appointment and engage them in HIV care by assisting patients in removing barriers that keep them out of care through the provision of tailored interventions. The Retention in Care Specialist will provide quality and comprehensive HIV Health Education, HIV testing, and HIV prevention education services to clients at high risk for infection or transmission of HIV. Core responsibilities include linking clients that have not attended an HIV medical appointment, implementing ARTAS, and maintaining clients engaged into care.

 

Responsibilities

  1. Provide excellent customer service at all times, to internal and external customers.
  2. Implement ARTAS model to overcome barriers for clients resisting care
  3. Participates in comprehensive training sessions (including monthly clinical supervision) for prevention.
  4. Coordinates AltaMed’s HIV Prevention activities and events, as directed by the Program Supervisor/Manager.
  5. Coordinate HIV Prevention schedules and events in conjunction with the HIV Supervisor/ Manager.
  6. Represents AltaMed HIV Services at community meetings and coalitions.
  7. Assists in the development of HIV Prevention presentations for internal and external meetings, conferences, and trainings.
  8. Review HIV health education materials for content, layout, cultural-sensitivity and linguistic appropriateness
  9. Providing HIV/STI Counseling and Testing.
  10. Clearly and accurately completes required documentation for MCC program forms such as data sheets, assessments, etc.
  11. Interface and collaborate with diverse group of health care professionals and outside community resources to coordinate referrals/linkages and patient care.
  12. Participates in regular internal audits to ensure that program documentation, charts and reports are accurate and complete on a monthly basis.
  13. Promotes HIV/STI Prevention services through in-service presentations to AltaMed staff and other agencies.
  14. Participates in program, departmental and division staff meetings.
  15. Ensures the maintenance of accurate activity logs and program reports and monitors and submits timely progress reports in addition to monitoring the grant’s HIV quality measures.
  16. Works collaboratively with Grants Management and Development team to develop and submit proposals to expand HIV Prevention programming.
  17. Knowledgeable about HIV/AIDS and current resources available
  18. Performs other related duties as assigned.

Qualifications

  1. High School Diploma required. Bachelors Degree in Science in Health Science with an emphasis in Community Health Education or related field or equivalent work experience preferred.
  2. A minimum of 5 years experience in a community HIV Prevention program or human service-related field, such as social work, psychology, nursing, counseling or health education
  3. Trained in Motivational Interviewing and demonstrates a capacity to deliver client-centered counseling.
  4. Demonstrated skills in Presentation and Group Facilitation.
  5. Applicant must demonstrate skill (through education or experience) in counseling, crisis intervention, critical reading and writing as it relates to implementation of program goals and objectives.
