Overview

This position is responsible for compiling, processing and maintaining patient medical records organized and updated in a manner consistent with medical, administrative, ethical, legal and regulatory requirements of the healthcare system, including medical histories, reports of X-Rays, laboratory tests, specialty consultations, treatments plans and other examinations. Health Information Representatives are also responsible for filing and storing records as well as processing requests for use & disclosures of PHI in accordance with health requirements and standards.

Responsibilities

  1. Answers telephone and assists callers in a pleasant and professional manner.
  2. Responsible for accurately inputting diagnostic test/procedure results into the Electronic Health Record (E.H.R.) application within established timeframes.
  3. Accurately scans health information into P/C/P E.H.R.’s chart.
  4. Indexes health information accurately by adhering to Four-Point-Check identity verification protocol.
  5. Responsible for ensuring Authorizations for Use & Disclosure of PHI are complete, properly fill-out, logged and processed timely as per Altamed’s policy.
  6. Adheres to minimum necessary standards when processing Treatment, Payment and Healthcare related requests to accomplish the intended purpose of the use, disclosure or request.
  7. Sort and distribute incoming mail.
  8. Process and stamping outgoing mail.
  9. Review and process incoming health information and updates E.H.R. pending orders.
  10. Reviews system to verify/process site lab results that failed to post to P/C/P record.
  11. Performs all other duties as assigned.

Qualifications

  1. High School graduate or GED certification or certificate from health care related training program required, e.g. Medical Assistant, Medical Management, Administrative Assistant .
  2. Some general HIM related experience preferred.
  3. Minimum of one year experience with electronic medical records preferred.
