Under the direct supervision of the Manager of Behavioral Health Services, the primary function of the Case Manager (CM) is to provide referral management for Substance Abuse and Behavioral Health patient population. The CM I will follow the program’s Scope of Work to achieve the program goals through the screening of patients, referral organization, referral management, appropriate linkage of care and record keeping.



  1. Provide Referral and case management services to Substance Abuse and Behavioral Health patients.
  2. Document all patient communication and outcome of referral and case management.
  3. Research, Refer and inform patients of services available to them outside of the scope of the program.
  4. Assist patients in navigating through the system for higher level of care
  5. Participate in community meetings
  6. Coordinates appointment with clinician for Substance Abuse or Behavioral Health Services.
  7. Assists patients with appropriate linkage to community agencies.
  8. Assist with Answering incoming calls to the Behavioral Health Department.
  9. Performs all other related duties as appropriate and/or assigned.


  1. Bachelor’s degree in Social Services, Sociology, Human Services, Counseling or a minimum of 5 years working in health care assisting patients with case and referral management and linkage to care.
  2. Current CPR/First Aid Training certificate required.
  3. Bilingual English/Spanish or English and other predominant language of population served preferred.
