Under the direction and supervision of a physical therapist, this position promotes and maintains health by providing physical therapy services to help patients recover from mobility issues.
Implement established interventions based on the assessment/plan of care as directed by the Physical therapist.
Obtain data related to the interventions provided, and make modifications either to progress the patient/client as directed by the physical therapist or to ensure patient/client safety and comfort.
Assist the physical therapist in the treatment of individuals who have medical problems or other health-related conditions that limit their abilities to move and perform functional activities in their daily lives.
Administers physical therapy treatments by utilizing applicable physical agents (i.e. moist packs/heating pads, cold packs, TENS, EMS, ultrasound machines); under the direction and supervision of the physical therapist.
Performs therapeutic procedures by administering, instructing, encouraging, and assisting patients in performing exercises, physical activities, ambulatory /functional activities, and daily-living activities); under the direction and supervision of the physical therapist.
Notify the physical therapist of record; document in the patient record any change in the patient's condition not within the planned progress or treatment goals, and any change in the patient's general condition.
Provide information to the patient about the proposed intervention, its material risks and expected benefits and any reasonable alternatives.
Collaborate with physicians, nurses, social workers, and other health care workers on patient’s integrated individualized plan of care. Contributes to team effort by accomplishing related results as needed.
Document in accordance with AltaMed PACE departmental guidelines and within the established time frames.
Maintains patient confidence and by keeping information confidential.
Protects patients and employees by adhering to infection-control policies and protocols.
Complies with federal, state, and local legal and professional requirements.
Perform all other related duties as assigned.
Physical Therapist Assistant license issued by the State of California Physical Therapy Board.
At least 1-2 years of experience working with the frail elderly and disabled population.
