Under the direction of the Medical Director of Quality, this position is responsible for assisting in design, implementation and evaluation of AltaMed’s quality program and improvement strategies and to oversee data collection, analysis, evaluation and reporting for both internal purposes and external entities. Function as Program Manager for QPID grants and assist in drafting quality related grant applications. Liaison to all corporate departments and external networks to ensure efficient workflows, improve quality of care documentation and outcomes and ensure accurate quality reporting.



  1. In addition to supervisory duties, fulfill duties of a Clinical Quality Analyst (see job description) for a set of assigned clinics

  2. Supervise QPID’s quality analysts assigned to supporting PACE sites and FQHC clinics in primary care and dental services. Provide feedback and coaching to Clinical Quality Analysts

  3. Supervise Clinical Outcomes Analyst in prioritizing and managing work orders related to NG data capture, specification/report modifications, quality reporting software(s), and annual HEDIS/P4P code updates.

  4. Plan and coordinate collaboration with Technical Training Unit for for EHR documentation training and advise Director of Quality Improvement on training needs and improvement efforts.

  5. Assist in creating quarterly action plans for the enterprise to improve strategic quality goals

  6. Oversee QPID presentations for monthly and quarterly FQHC quality related meetings including provider meetings, Site Medical Director Meetings, MPQR, DPQR, Dental Provider, Back Office Supervisor, and Dental Supervisors.

  7. Oversee the provision of processed data analytics to leadership, providers, and clinic staff, on performance improvement and quality of care and service intervention efforts.

  8. Work with the Director and Medical Director of Quality to identify and address quality of care and patient experience issues.

  9. Track and trend specified patients/population/conditions that support the clinical and care management of these patients.

  10. Support quality improvement projects and initiatives –assist with planning of the monthly Clinical Quality Improvement Committee (CQIC)

  11. Supports QPID in service, process and quality improvement efforts to ensure standardization and integration between programs.

  12. Participate in the department’s strategic planning process to develop innovative responses to address current and future needs and challenges through planning and management of QI.

  13. Distributes and interprets reports related to the respective programs supported.

  14. Keep abreast of industry changes and serves as a subject matter expert on programs affecting quality, population health, and incentive programs.

  15. Performs process improvement analysis and prepares/presents related documents for health care clinics. Support the development of QI, project tools,

  16. Implement performance improvement tools to support clinic outreach activities, track data gaps, and support Health Care Disparities, HEDIS, CMS StarP4P and other required health plan reports.

  17. Provides technical assistance, consultation, coordination, facilitation, and education activities; evaluate departmental programs and services in collaboration with customers and key stakeholders.

  18. Contributes to the departmental policies and procedures and contribute to the evaluation of department programs and annual reports as needed.

  19. Performs all other related duties as assigned.



  1. Bachelor's Degree in Business Administration, Health Administration, Social Worker, Public Health or equivalent experience to the role expectations. Master’s Degree is preferred.

  2. Advanced certification or licensure in health care or process improvement is desired (LVN, RN, SW, Project Management, LEAN, or Six Sigma).

  3. A minimum of 3 years prior experience in an acute and ambulatory healthcare environment performing process improvement duties.
