Responsible for participants assigned to the Social Work team and works closely with the provider and nurse to coordinate services and advocate for participants. Responsibilities include, but are not limited to supporting initial assessment, quarterly and semi assessments and annual re-assessment within the scope of practice; conducting family conferences, assisting with community referrals and regular contacts with participants. Provides supervision and direction for the day-to-day social work services.



  1. Perform all disciplinary counseling sessions, performance evaluations, PTO, vacation requests, sick leave requests for department staff.
  2. Supports initial and semi-annual assessments, quarterly notes and develops care plans for assigned team/participants of Senior Care Services Program(s) and complete within required timeframe set by state guidelines and within their scope of practice.
  3. Interview and screen program participant’s family members and caregivers to identify additional need for psychotherapeutic intervention and inform immediate supervisor.
  4. Provide information and referral to program participants and family and assist with coordination of services through public, private and social services agencies while acting as an internal and external liaison.
  5. Provide oversight and supervision on all participant disenrollment and discharges from the PACE program.
  6. Coordinate and manage all satisfaction surveys and reports as requested by PACE program.
  7. Provide oversight and coordinate completion of Incident reports and Grievance reports to meet the deadline as set by State guidelines.
  8. Works with Care Coordination/Management Team (RN, OT, PT, Activities) by taking lead in investigating non-clinical psycho-social problems as identified by the multidisciplinary team and document in NextGen and appropriate electronic support systems.
  9. Coordinates family conferences, attends team meetings with provider and R.N. and documents all participant and family contacts in accordance with program documentation guidelines.
  10. Perform participant psychosocial tasks as necessary by need and program Administration, maintaining a caseload of up to 120.
  11. Participates in daily Inter Disciplinary Team meetings and other mandatory meetings.
  12. Maintains mandatory contact with participants within the assigned caseload and develops and conducts specialized therapeutic groups at least one time a month.
  13. Provide home visits if unable to contact by phone and provide feedback to team and administration.
  14. Conduct monthly staff meetings to for all Social Workers to improve quality of service, enhance skills in the management of the case loads and submits mandatory reports on a timely manner.
  15. Works to ensure compliance of social work staff to program standards and regulations and supports the development of the appropriate program policies and procedures.
  16. Perform other related duties as assigned.



  1. Masters’ degree in Social Work from an accredited university required.
  2. Minimum two (2) years’ experience in supervision or management required.
  3. CPR/First Aid certificate required.
  4. Bilingual English/Spanish, English/Cantonese/Mandarin, or another second language strongly preferred.
  5. One year experience in case management/social advocacy with geriatric population preferred.
