    The Site Medical Director-PACE has overall responsibility for the clinical patient care at PACE site(s) he/she is delegated to oversee. He/she addresses clinical, programmatic, medical, and administrative issues at the site in collaboration with the Clinic Administrator and Nurse Administrator. The Site Medical Director-PACE or designee actively participates in the Interdisciplinary Team and directs the medical care decisions.

     

    Responsibilities

    Managerial Skills: 15%

    1. Supervise all medical services offered by the program and provides primary care at the PACE site(s) delegated to oversee.
    2. Reviews physician and mid-level professional staff performance on quality, patient experience and appropriate resource use at the site no less than monthly, and performs individual provider evaluations no less than annually.
    3. Orient all new physician and mid-level providers, and oversees daily coverage.

    Financial/Budgetary Skills: 15%

    1. Assists in managing and monitoring the cost of hospitalization, specialty and ancillary services and identifies, implements and measures areas for improvement.

    Quality/Patient Care Responsibilities: 65%

    1. Management of on-site modified panel based on clinic needs and resources, works with IDT to provide full spectrum services to paneled patients according to PACE regulations. Work collaboratively with Mid-Level extender providers where appropriate to provide panel management of internal site panel.
    2. Works in collaboration with pharmacist-PACE to oversee and monitor clinic stock medications, dispensary medications, and individual patient prescribed and stored medications onsite.
    3. Initial review of PACE site Incident Reports to monitor for quality of care issues, process improvement opportunities, and site training needs, to be sent to Regional Medical Director PACE for final sign off.
    4. Oversees clinic services, develops systems to track and monitor urgent care services onsite, i.e. IVF, injections, Antibiotics, procedures.
    5. Oversees the management of participant’s medical situations, and oversees the participant’s use of medical specialists and inpatient care.
    6. Assist in data collection analysis and preparing reports regarding clinical service delivery.
    7. Provide active leadership in UM/QM committees to monitor quality of care, patient experience and appropriate resource use indicators and assist in the identification and implementation of appropriate interventions.
    8. Ensure that all committees are effectively accomplishing goals established in the QM/UM Program, and all activities are clearly documented within minutes and/or reports.
    9. Collaborates with Quality and Process Improvement and Corporate Medical Management Department to enhance quality of care delivery among all health professional services.
    10. Work with Site Physicians to develop coordinated process flows between hospitalist and Physician/Clinic Staff.
    11. Work with Site Physician to develop a collaborative process for working with facility discharge planning staff and contracted facilities.
    12. Ensure that chart audits are being conducted, identify problems and provide guidance for resolution. Work through QM/UM to audit process and outcomes.
    13. Work to develop, implement and monitor clinical guidelines that are appropriate for the assigned population.
    14. Work with Site Physicians to ensure that pharmaceutical care meets State requirements, and professionally recognized standards of practice.
    15. Ensure that effective drug reviews and pharmacy policies are implemented.
    16. Ensure that peer review is performed regularly for hospital based providers and clinic based ancillaries.
    17. Ensure that contracted pharmaceutical consultant services are provided timely, reports are adequate, appropriate and that there is follow-up.

    General Responsibilities: 5%

    1. Responsible for site coverage in clinic emergencies, and assists in the development of site clinical policies and protocols.
    2. Represent AltaMed to external providers and organizations regarding patient management and clinical issues as appropriate.
    3. Work with Corporate Office Staff to develop system to prepare agendas and attachments for committee meetings, and to maintain accurate, signed, minutes for review.
    4. Assist in the development and implementation of the Corrective Action Plans for periodic audits by outside agencies.
    5. Performs other related duties as assigned. 

    Qualifications

    1. Physician with an active unrestrictive CA license to practice medicine by the Medical Board of California and DEA License required.
    2. Prior experience in a leadership or management role strongly preferred.
    3. Previous experience with computer-based systems required; Electronic Medical records experience preferred.
    4. Education, training and experience necessary to meet the underwritten requirements for inclusion under AltaMed’s malpractice insurance coverage.
    5. Current CPR and First Aid certificate required.
    6. Bilingual: English/Spanish/Mandarin/Cantonese preferred depending on location.
