  • Overview

    Under the supervision of the site Center Manager, the Care Center Supervisor is responsible for supporting all aspects of selected operational areas at the assigned site. These operations may include but not limited to clinical support, participant personal care, recreational therapies, administrative, nutritional, participant transportation and/or day center activities, all of which ensure the efficient and effective delivery of health care and social services at the AltaMed PACE center. He/She will be accountable to support the achievement of efficient operations in accordance with licensing, regulatory standards and company policies and procedures. Additionally, he/she will contribute administrative skills and abilities to the staff selection and retention process, annual budgets, process improvement efforts and care coordination with internal and external resources. 






    1. In collaboration with the Center Manager, responsible for the planning, coordinating, implementing and monitoring of the PACE day-to-day activities, operations, physical plant, supplies and equipment.
    2. Responsible for the coordination and supervision of assigned departments.
    3. Support and/or assist with process improvement efforts with other department supervisors to achieve PACE operation goals by setting priorities, reviewing budget guidelines, implementing job assignments and expectations, and monitoring facility and staff safety.
    4. Supports the Center Manager and/or leadership team in the facilitation, leadership and monitoring of the Interdisciplinary Team (IDT) meetings, including oversight of assigned IDT action items, care coordination and utilization of health care services.
    5. Represents center management by attending required PACE Committee meetings, including QM, UM, Safety, and Participant Advisory as assigned.
    6. Assists in the development and implementation of the annual budget, and reviews monthly operation expenses to assure budgetary compliance and identifies any budget variations as appropriate.
    7. Assists the Center Manager with managing and monitoring PACE center quality systems, including but not limited to: department policies and procedures, clinical outcomes, cycle time, productivity, and participant satisfaction for continuous service improvement.
    8. Assists the Center Manager with developing appropriate PACE center processes to improve work efficiency and quality, reduce operational costs and maintain staff accountability by performing and auditing work flow analysis with existing departments.
    9. Assists with regulatory management, including compliance in all areas of program operations, performance against national benchmarks and established service standards, and preparing the necessary Corrective Action Plan (CAP) as needed and in a timely manner (JCAHO; CMS; DHCS, Clinic; Dept. Of Education; DHCS-Office of Long Term Care; AltaMed).
    10. Collaborates with the Center Manager to ensure compliance in all areas of personnel management including recruitment, timely employee evaluations, competency assessments, annual physicals and TB clearance, daily coverage, training and staff development, CEU’s, and disciplinary actions in accordance with company policies and procedures.
    11. Performs other related duties as assigned.


    1. Experience working collaboratively with some and/or all the following disciplines: nursing, social work, psychology, recreation, occupational therapy, physical therapy, speech therapy, dietetics, and/or gerontology required.
    2. Minimum of two years’ experience in supervisor or management position in healthcare or related field required.
    3. Bachelor’s degree in Social Work, Nursing, Psychology, Gerontology, Health Care Administration or other health care discipline preferred. License required if he/she has clinical degree.
    4. Experience in process or quality improvement projects or activities preferred.
    5. Bilingual: English/Spanish/Mandarin/Cantonese depending on site location preferred.
