    Under the direction of the Clinic Administrator (HIV Services), the Quality Improvement Specialist II is responsible for providing support services to HIV Services in the areas of root cause analysis, process improvement techniques, and working with corporate operations staff in the definition and enhancement of existing processes in order to improve HIV Services program efficiencies and patient outcomes. Responsible for the administrative and quality management, planning, development, implementation and evaluation/assessment of clinical care services/programs. Reviews and performs medical records data review and analysis related to quality care issues; coordination with the HIV Services grant programs to sustain improvements in quality measures and service outcomes to meet the demands of regulations set forth by the Ryan White Parts, prevention, manage care compliance, and other grant funded programs. This position coordinates with internal and external customers to meet deliverables throughout the year.

    Responsibilities

    1. Performs ongoing monitoring for assigned HIV and Prevention Sites (Daniel V. Lara (DVL) clinics) to include: HIV Performance Measures and Quality Indicators.
    2. Performs routine chart reviews.
    3. Assists in collecting, monitoring, and reporting of HIV quality chart reviews by program by month and provide quarterly analysis.
    4. Provides quality assurance assistance, consultation, coordination, facilitation and educational activities to customers and key stakeholders.
    5. Gathers demographic, diagnosis, and utilization, clinical quality, and performance data for analysis and reporting for QM.
    6. Conducts workflow analysis and process mapping to identify areas for improvement.
    7. Implement performance improvement tools to support HIV program sites processes, outreach activities, track data gaps, and decrease health care disparities.
    8. Collect, analyze, interpret and report inclusive presentations and display data that improve processes and patient care for QM.
    9. Assists in implementation of best practices across HIV program sites.
    10. Provides support to the providers, HIV and prevention staff on measure specifications and quality improvement techniques.
    11. Leads data analysis projects, including use of quantitative analytics for strategic initiatives.
    12. Monitors processes and conducts focused care reviews/ audits identified through event reports, staff referrals/inquiries, quality assurances and customer concerns in association with quality indicators and measures.
    13. Assists Clinic Administrator to work with sponsors, regulatory agencies, internal and external stakeholders, to support quality initiative deployment, issues resolution, change/manage with meaningful data driven information.
    14. Acts as an administrative liaison and resource with corporate compliance and department management team.
    15. Participates and collaborates with HIV Services Management team as a team member in preparation of Annual Programmatic Reviews (Audits).
    16. Keeps abreast of industry changes and shares information with Quality Improvement team on programs affecting quality, and population health.
    17. Performs other duties as assigned.

    Qualifications

    1. Bachelor’s Degree prepared in (Nursing, Social Work, and Healthcare Management) or Licensed as an RN in the State of California preferred.
    2. A minimum of two years’ experience in process improvement and/or other quality management/improvement related experience preferred.
    3. Knowledge of Ryan White HIV/AIDS Program Legislation preferred.
    4. Advanced certification or licensure in health care or process improvement is preferred (Lean or six sigma).
    5. Prior experience a minimum of one year with an Electronic Health/Medical System (E.H.R/E.M.R) -NextGen preferred.
    6. Current CPR/First Aid Training.
