    Under the direction of the Pharmacy Director, this position will provide clinical pharmacy support for AltaMed providers in managing the prescription refill authorization process using a prescriptive authority protocol.The pharmacist will provide ambulatory care services using approved guidelines, protocols, and clinical knowledge to deliver enhanced refill and medication management services and improve operational efficiencies. The position will also oversee pharmacy technicians at the Medication Adherence and Centralized Prescription Renewal Unit. This team will be based at the Monterey Park office and will receive phone call, faxes, and other forms of communication to process requests for prescription renewal. There is no a pharmacy located at this site, and handling of drugs is not within the scope of this role.

     

     

    Responsibilities

    1. Utilizes clinical and technical knowledge to develop policies and procedures to improve prescription refill services, increase medication-related quality metrics (e.g. HEDIS measures, Medicare 5 STAR Ratings), handle effective therapeutic substitutions, and improve refill procedures for high cost specialty medications.
    2. Reviews patient history and labwork, assesses current drug therapy for appropriateness, compliance, duration and duplication of therapy
    3. Identifies, resolves, and prevents potential and actual medication-related problems including: untreated indications, improper drug selection, subtherapeutic dosage, overdose, failure to receive medication, adverse drug reactions, drug interactions, and medication without indication.
    4. Provides documentation in medical record and communication to prescriber as per prescription refill protocol.
    5. Under protocol, initiates, adjusts, monitors, and recommends changes in drug therapy based on information obtained through patient interviews, objective monitoring parameters, review of drug profiles and the patient medical record.
    6. Under protocol, coordinates, interprets and orders laboratory monitoring of drug therapy, including serum drug levels, electrolytes, and other approved drug-related laboratory testing.
    7. Provides patient monitoring for the purpose of evaluating and optimizing patient outcomes, drug utilization and patient compliance, detecting and/or minimizing ADR's, eliminating unnecessary drug use and duplication.
    8. Supervises and works with pharmacy technicians to manage daily workflow
    9. Provides drug therapy information to patients, including patient education related to the individual's disease state, predictable actions, side effects, reasons for proper compliance, and any special instruction.
    10. Assures ongoing performance improvement of the pharmaceutical care services provided.
    11. Provides functional direction to care management team, analyze and resolve problems in order to assure high quality and efficient workflow to improve health outcomes.
    12. Provides education to providers and staff via presentations, newsletters, formal utilization reviews, and one-on-one interactions.
    13. Performs other duties as required.

    Qualifications

    1. Graduate of an ACPE accredited college of pharmacy with a Doctor of Pharmacy degree and a California pharmacist license required.
    2. Minimum two years as a practicing Pharmacist also encouraged.
