  • Overview

    The Social Work Care Manager is responsible for the evaluation of the potential client as a total person and identifies the limitations which impede their daily living.  Furthermore, the Social Work Care Manager is responsible for case recording and reporting on a timely basis, and participating in care plan development and facilitation of the coordination of social/health services accordingly.


    1. Provides a comprehensive psychosocial assessment of the client’s situation and needs.  An assessment must be conducted on an annual basis projected from the client’s initial assessment date. 

    2. Responsible for reviewing the assessment instrument for completion and accuracy of the data obtained during the assessment interview.

    3. Participate in a Care Planning Conference, develop the care plan and determine service needs in conjunction with the Supervising Care Manager/SCM. 

    4. Responsible for the coordinating, referral and monitoring of social/health-related services; this includes coordinating the care plan services by linking the client with the corresponding service providers, to monitor and ensure the timelines and quality of service provision; and identify and correct problems with service system that prevent the client from receiving needed services. 

    5. Will consult with physicians and other health professionals regarding the client’s health status as deemed necessary and obtain care plan approval from the physician whenever possible.

    6. Provide home visits to the clients, so that an adequate level of functioning can be maintained.

    7. May provide necessary information to the client and family concerning his/her health condition(s), medication(s), reaction(s) diet and prevention of complications; will also be available to interact with client and family with the goals of providing support, understanding, direction, resolution of problems when possible.

    8. Will function as an advocate for the client in relation to the service system to enable the client to receive existing services or to advocate for non-existing services that are needed.

    9. Provide supportive counseling where appropriate in order to enable the client to accept and utilize services accordingly.

    10. Maintain progress notes in a timely and relevant basis.

    11. Responsible for monitoring and not exceeding Waived Services budget on a monthly basis.

    12. Performs other related duties as assigned. 


    1. Bachelor’s degree in Social Work or related field required.

    2. A minimum of two years care management experience working with geriatric population required.

    3. Bilingual English/Spanish highly preferred.
