    Responsible for the general oversight and management of nursing at all AltaMed Senior Care PACE Centers. Serve as a key resource and subject matter expert for nursing, home care support services, and clinical support to center leaders, and lead PACE Quality initiatives. Provide leadership for nursing functions while working collaboratively with the PACE Organization’s Administrators, Clinical Programs Director, Medical Director, AVP, Senior Care Services, and VP, Senior Care Services to ensure achievement of PACE strategic goals. Represent nursing practice within the interdisciplinary team approach to ensure implementation of quality, cost effective, standardized and efficient processes which support coordination of participant centered care goals at all PACE sites. Support recruitment, onboarding and training to ensure nursing clinical staff is knowledgeable, skilled and competent to provide quality participant care in the PACE model.

     

    Responsibilities

    1. Accountable for all aspects of nursing care delivered by AltaMed PACE centers which may include in the center, community or home.
    2. In collaboration with Center Managers, Administrators and Operations Leadership, responsible for coaching and developing staff in support of technical and professional growth.
    3. Develop, implement, audit and communicate clinical practice and clinical policies and procedures for best practices, evidence based practice standards, and evaluation of nursing staff at each PACE location (RN, LVN, CNA, MA). (e.g. personal care, direct participant care, care management and home care coordination).
    4. Develop and implement operational and nursing policies and procedures to ensure participant, provider, and employee satisfaction while promoting safety and retention. In addition, provide clinical support to all PACE Centers to ensure that all policies, safety protocols, Clinical Guidelines and facility requirements are met.
    5. Responsible for ensuring nursing policies and procedures are current and properly communicated to all staff and procedures are performed within the scope of nursing practice. Assure all nursing functions within the PACE Center are optimized, efficient and support adherence to the participant’s plan of care.
    6. Ensure accountability and standardization of nursing practice for all PACE Center team performance measures and outcomes.
    7. Ensure compliance and scope of practice as established by the California Nursing Practice Act and Board of Registered Nursing.
    8. Serve as a liaison between operations, quality and medical director to improve care coordination and ensure participant/family satisfaction.
    9. Responsible for PACE clinic efficiency and ensures all implemented process improvements/ projects or initiatives are compliant and maintained.
    10. Work collaboratively with the Center Leadership, Administrators, and Clinical Programs Director in planning and implementing standardized clinic and center processes.
    11. Develop and manage performance improvement projects in coordination with Senior Care Services Quality and Administrative Leadership.
    12. Collaborate with other disciplines within the PACE Model (i.e. Rehabilitation, Social Work, Nutrition) in providing quality care, interdisciplinary collaboration and compliance with state and federal laws and PACE Program regulations.
    13. Support regulatory audit activities in collaboration with Operations Leadership, Medical, Quality and Center Leadership. Develop and implement corrective actions plans for areas of responsibility.
    14. Work with Operations Leadership to meet goals & objectives for the strategic plan of the organization. Participate in enterprise strategic planning and ensure communication and expectation setting is communicated to staff.
    15. Ensure timely and accurate submission of all required reporting, such as encounter data, quality and incident reporting to the appropriate regulating bodies.
    16. Support each PACE center’s clinical leadership nursing team’s staffing strategies as necessary.
    17. Assist with the recruitment, onboarding and orientation of nursing leadership and adherence with Human Resources practices of hiring, performance management and coaching (PIP), and termination of clinical nursing leadership in collaboration with Center Managers and PACE Administrators as needed.
    18. Ensure the recruitment, screening and hiring of nursing and senior care support staff to ensure clinical competence. Responsible for identification, development and implementation of skills competencies for all nursing staff in the PACE Centers. Help lead/train nursing managers, supervisor, licensed nursing staff, and nursing support staff.
    19. Manage clinical orientation and onboarding for nursing and clinical support positions (RN, LVN, Medical Assistant, CNA). Work collaboratively with AltaMed University, Technical Training University and Human Resources personnel to ensure orientation and onboarding for PACE staff is timely, appropriate and comprehensive to meet the needs of the PACE program.
    20. Serve as the nursing representative for PACE on enterprise clinical committees, such as Pharmacy & Therapeutics, HR training for clinical disciplines, etc., as well as external representative for AltaMed PACE as needed.
    21. Serve in a leadership role in preparation for regulatory audits, and as a subject matter expert to ensure adherence to regulatory requirements.
    22. Perform all other related duties as assigned.

    Qualifications

    1. Registered Nurse with a current California State Board of Nursing License coupled with a minimum of ten (10) years Registered Nursing experience and a minimum of five (5) years advanced practice or above nursing leadership experience.
    2. Master’s degree in Nursing required.
    3. One (1) plus year of experience working with the frail or elderly population as required by CMS PACE regulations. Additional experience working with geriatric patient populations preferred.
    4. Bilingual English/Spanish preferred.
    5. Current BLS CPR/First Aid certificate required. ACLS preferred.
