Assist with program recruitment, eligibility screening, intake, and coordination of patients of AltaMed’s obesity prevention & wellness programs specific to location(s). This Wellness Clinical Assistant (WCA) functions as an AltaMed ambassador, is welcoming and attentive to the needs of all patients and creates a positive patient experience by managing expectations while demonstrating compassion and respect. The WCA will assist in the provision of direct patient care for program participants of wellness programs by performing routine program care procedures and technical/clerical support services in the clinical setting. The position performs this role by building relationships with patients, listening to them, and utilizing Motivational Interviewing skills to foster positive environment for participants.
The position will also demonstrate compassion and respect and will be proactive in managing patient’s expectations. In addition, the WCA will document all specialties in the Electronic Health Record (EHR) and assist with more administrative responsibilities that include providing feedback on Back Office processes to the Nursing Supervisor/Manager, Clinic Administrator and Department Supervisor.
Program Workflow: The WCA will lead secondary MAs on clinic days and ensure smooth workflow and efficient patient back office processing; weekly reports to program management.
Program Tracking/Reporting: The WCA will assist with case management of patient progress throughout obesity prevention program including assisting provider in monitoring patient metrics progress during each phase of the program. The WCA will update BMI and other metrics at program milestones, and update provider on patient progress towards 12-month completion. S/he will manage client files throughout all stages of the program, alerting program team of discontinuing patients.
Enrollment: The WCA will assist with opening tasked referrals for the program.
Referral Management: Manage referral inbox and program enrollment; assess readiness to enroll, barriers to completing the program, language assessment, and/or tentative action plan to prepare for enrollment. Task referring provider on patient contact outcome or lack thereof. Schedule proper clinical visit type based on patient contact. Management of enrollment to meet cycle target; increase outreach and recruitment as needed to meet targets.
