  • Overview

    Under the direction of the Health Education & Wellness (HEW) Supervisor, the Wellness Program Coordinator is responsible for the overall implementation and success of various wellness programs, targeting pediatric obesity risk-reduction, pre-diabetes, diabetes prevention/management and more at the site level. Specific responsibilities to include: monitoring participant/patient screening process, enrolling program participants, updating/managing patient’s files throughout all program phases, overseeing scheduling of sessions, tracking and monitoring patient’s progress, and ensuring that data collection and reporting processes are accurate and valid. He/she will ensure all wellness programs are following proper protocol adherence at all times. S/he will function as an AltaMed ambassador, welcoming and attentive to the needs of all patients and helping to create a positive work environment and patient experience by managing expectations and demonstrating compassion and respect. Program Coordinator is responsible for relationship building with patients; specifically utilizing Motivational Interviewing skills to foster positive environment for participant.

    Responsibilities

    1. Effectively works with the HEW Supervisor to coordinate new or existing wellness program implementation and/or monitoring under the direction of the HEW Director/Manager.
    2. Program Coordinator will enter/oversee the collection and summation of reports (weekly/monthly/quarterly) tracking attendance, attrition, outreach, enrollment, patient satisfaction, behavioral modification, BMI and BMI percentile, patient metrics, and other program tracked data as requested for all programs. He/she will share reports with clinic leadership and department management.
    3. Wellness Program Coordinator will work with the HEW Supervisor to assess efficiency of program(s) through concreate program evaluation assessments and ensure that data collection and reporting processes are accurate and valid.
    4. Updates patient’s program file throughout all phases and tracks their enrollment status (active or dropped out) and provides case management like assistance when needed.
    5. Ensures adequate inventory of necessary program supplies.
    6. Collaborate with front office supervisor in verifying, opening and closing of template as required by programs. Manage proper scheduling on program template including follow-up with no show appointments, new enrollments, and maintenance continued care.
    7. In preparation for program operating day(s) he/she will verify insurance for existing and new patients, while assisting with participant phone call reminders for all onsite wellness programs. He/she will effectively manage assigned patient flow during program operating days to minimize delay in processing.
    8. Processes patients efficiently and documents on Practice Management System per policy for all wellness programs.
    9. Responsible for communicating program upgrades to clinical teams in a timely manner while ensuring content of message is accurate and uniform across all sites.
    10. Wellness Program Coordinator will work closely with clinic staff to meet targeted enrollment and retention efforts. Program coordinator will increase efforts to meeting target as needed.
    11. Functions as a participant advocate ensuring that participant concerns are resolved or addressed expeditiously.
    12. Cultivates and maintains relationships with referral sources and multi-disciplinary team that includes but is not limited to medical, dental, prenatal, providers, case managers, outreach, patient service center and outside sources.
    13. Performs other related duties as assigned.

    Qualifications

    1. High School diploma or GED required; Bachelor’s degree preferred.
    2. English/Spanish required due to population served.
    3. CPR/First Aid certificate preferred.
    4. Proficiency in MS Office applications, including MS Work, MS Excel and Outlook required.
    5. Experience working with community-based programs a minimum of 4 years required.
