This position is responsible for leading and managing front office operations dedicated to meeting the expectations and requirements of internal and external customers by ensuring the delivery of the highest quality patient experience by partnering with site management, measuring progress against goals, identifying trends and implementing process improvements.

The Coordinator, Scheduling position is responsible for scheduling and coordinating off-site specialist appointments for urgent, routine and follow-up appointments in accordance with the approved authorizations, scheduling and turnaround time objectives.  This position works closely with members of the IDT team to coordinate services and advocate for participants specialty needs. The Scheduling Coordinator advocates for participant’s specialty needs, and is an integral part of the care team, working as a liaison between the participant, primary care physician (PCP), specialty providers and Utilization Management to ensure timely access to specialty care.

Under the general supervision and within the regulatory scope of practice, assist in the provision of direct patient care by performing routine patient care procedures, technical and clerical supportive services in the clinic setting.  The M.A. may administer intramuscular, subcutaneous or intra-dermal injections and may perform skin tests, administer oral medication, venipunctures and skin punctures for blood specimens.

The RN Case Manager provides daily care coordination, case management, coaching, consultation and intervention to patients with one or more chronic diseases. The RN Case Manager is responsible for identifying said population via provider/clinic referral, utilization management referral, disease registry reporting mechanisms and patient self-referral. This position will also provide case management to patients who are admitted to the hospital and those patients who may need to be enrolled in ambulatory case management. Works as part of an interdisciplinary care team coordinating social work and mental health counseling, psycho-social support services, in-home support, legal services, skilled nursing, home health, etc. The RN Case Manager effectively collaborates with members of the interdisciplinary care team and with the physician in the clinic. The RN Case Manager performs telephonic face to face assessments on assigned patients. The RN Case Manager serves as the leader of a cohort of LVN Case Managers, Case Manager Coordinators, and Social Workers providing Case Management to a specified population of patients. RN Case manager provides support and guidance to said cohort.

The Care Gap Coordinator functions as a gate keeper for Call Center tasks and coordinates patient flow for providers. Submits and monitors HEDIS referrals through AltaMed Net. Also provides technical/clerical support in the clinical setting as necessary.

Under the supervision of the Associate Medical Director & Clinic Administrator, provide medical services to clients, ensuring appropriate and professional medical services.

Provides primary care medical services to the patients of AltaMed. Manages assigned patient panel.

Under general supervision by the Supervisor, Nursing and oversight by the PACE Physicians, the Nurse (RN) Case Manager/Care Coordination Team Leaderhas the responsibility to case manage all PACE participants who are in the ambulatory setting. 

Provides administrative support to the assigned unit. Responsibilities may include: scheduling appointments, giving information to callers, dictation, composing memos, transcribing notes, handling multiple projects, and preparing and monitoring invoices and expense reports. May also research and create presentations, generate reports and represent management at meetings as needed.

Under the supervision of the IPA Medical Director, the IPA Physician Assistant (PA) will support IPA member clinics by providing medical services, clinical support for Medicare Health Assessments (MHA) and Initial Health Assessments (IHA), educational trainings, electronic health record (EHR) clinical support, and didactic refreshers. The IPA PA will help to improve quality of care, facilitate appropriate care utilization, support EHR coding and documentation, and ensure completion of needed medical services by being an advocate and champion for best practice standards in care delivery.

The Nurse Case Manager may provide daily care coordination, case management, coaching, consultation and intervention to patients with one or more chronic diseases. This position may also provide case management to patients who are admitted to the hospital and those patients who may need to be enrolled in ambulatory case management.  The case manager will be responsible for identifying (California Children Services) CCS cases, handle transfers, and retro reviews.  Works as part of an interdisciplinary care team coordinating social work and mental health counseling, psycho-social support services, in-home support, legal services, skilled nursing, home health, etc.  The Nurse Case Manager effectively collaborates with the hospitalist, the hospital nursing personnel, with members of the interdisciplinary care team and with the physician in the clinic.

The Network Engineer will be responsible to design, implement, maintain, and support AltaMed’s growing network infrastructure. This position will be part of a team that is responsible for designing and developing scalable, maintainable, highly available network architectures that meet business objectives and SLAs.

Reporting to the Director of Grants Management (DGM), the Research Grants Administrator (RGA) is responsible for coordinating, managing and supporting the pre and post activities of research grant activities of the AltaMed Institute for Heath Equity (AIHE), including coordinating and overseeing the administrative work of the Research Committee. The RGA monitors and ensures that standards and deadlines for all research related proposals are maintained, including incoming proposal partnership requests from academic institutions. The RGA reviews AltaMed proposals prior to submission for completeness, and ensures compliance with organizational standards, policies, and procedures. The RGA is the main liaison for internal and external Principal Investigators (PIs), ensuring they adhere to Research Committee requirements in a timely fashion. The RGA maintains records of research proposals submitted, and status of submissions, and is responsible for the preparation and management of proposals in accordance with funder’s requirements. The RGA negotiates contractual provisions with potential research partners, and is the liaison and single point of contact for all current or proposed collaborating research project owners. The RGA provides input to DGM, VP of Operations, and Medical Director for strategic decisions of the Institute. This position works with all levels of the organization in facilitating and coordinating cross-functional teams, obtaining information from all relevant sources, analyzing information and evaluating results to choose the best solution to resolve problems, using individual judgment to determine whether events or processes comply with laws, regulations, or standards. The RGA will be skilled in managing groups to work together to accomplish tasks; and supervises direct reports.  

Responsible for the administrative and business management of a mid-to-large clinic (30-50 FTEs) or multiple clinics (with a total of 30-50 FTEs), providing ambulatory patient care and direct planning, development and implementation to achieve organizational goals: Patient Experience, Quality and Population Health, Affordability and, Clinic Team Wellbeing. Working closely with site medical director, clinical and/or non-clinical managers and supervisors ensures daily operations achieve a high-quality, patient-focused primary care services for Senior Care, Adult Medicine, Pediatrics, Women’s Health, and some specialty services. The clinic director will lead engagement and development of all primary care clinic leaders and frontline staff to ensure clinic achieves safe, comprehensive, high quality, accessible, affordable and patient centered primary care across the continuum of care. The clinic director will comply with local, state, and federal requirements, inclusive of accreditation standards. The clinic director will collaborate with providers, nursing and front office leaders to develop and update evidence based clinical practice practices that are aligned with institutional policies and needed practice changes. The clinic director will report up to Associate Vice President (AVP) of Regional Operation and will be accountable to ensure clinic achieves budget, fiscal performance, and employee engagement. The clinic director will work closely with AltaMed departments to address clinic needs such as integrating new services/operations with provider and nursing evidence based standards, service priorities and quality performance/outcomes measures, as well as with enterprise goals and objectives. The clinic director is expected to collaborate with the patient access team and patient service center to ensure timely access to care. The clinic director will lead projects and PDSA (plan-do-study-act) models to begin and scale projects with regional support. This role will serve as a mentor for assistant clinic directors.

Functions as an AltaMed ambassador, is welcoming and attentive to the needs of all patients and creates a positive patient experience by managing expectations while demonstrating compassion and respect. The M.A. II will assist in the provision of direct patient care by performing routine patient care procedures and technical/clerical support services in the clinical setting. He/She will perform this role by building relationships with patients, listening to them and gaining their trust. He/She will also demonstrate compassion and respect and will be proactive in managing patient’s expectations. In addition, the M.A. II will document all specialties in the Electronic Health Record (EHR) and assist with more administrative responsibilities that include providing feedback on Back Office processes to the Nursing Supervisor/Manager. Will also assist in achieving department goals and objectives by providing support and guidance to the M.A. team as a mentor and trainer. If working at the Patient Service Center, will provide other duties over the telephone.
