The Patient Service Center Supervisor will be responsible for supervising call center staff to ensure consistency with appointment scheduling and call routing throughout the department. This position is also responsible for ensuring efficiency in the call center as well as monitoring performance. The position in addition to supervising will also be assisting the call center staff with all of their duties and responsibilities. This position will work closely with the Manager of Front Office Functions and Clinic Administrators with the overall tasks and to ensure standardization.

The Data Strategy Analyst is responsible for developing and maintaining multi-year Data & Analytics strategic plans and roadmaps to enable key business objectives. Highly visible across the enterprise, this role works with senior and functional leaders from across the organization to understand current business needs, anticipate opportunities for data & analytics integration, and develop strategic frameworks and capability roadmaps that support performance improvement. The data strategy analyst will collaborate with data & analytics technical teams to bridge business objectives to analytical products & solutions.
To be successful in this role, individuals must display an in-depth understanding of both technical perspectives surrounding data management & data analytics, as well as the business drivers & imperatives for data-driven solutions. This is a role with shifting responsibilities across a wide array of functions – data strategy, product & project management, business & data analysis, and analytics consulting / problem solving.
The Software Engineer position will be responsible for the analysis, design, development, testing, documenting and support of information systems. This role primary focus will be working with “SHARE” software system. SHARE system is in use by hundreds of internal users daily to process different crucial workflows. This role must be detail-oriented and able to communicate effectively across multiple departments. Must be comfortable gathering requirements, providing technical support, documenting architectural design, and creating reports for the executive team.

Under the direction of the Director of Health Plan Contracts, the Health Plan Contracts Manager assists in the development and management of payor contracts through contract negotiations, payor relationship maintenance, monitoring existing payor contracts, monitoring AltaMed responsiveness to payor communications and problem resolution when needed. The primary focus of this role is to initiate existing payor contract rate renewals on a consistent basis, and review and document payor contract amendments as needed. Identifies and recommends payor contracting strategies to reduce costs, limit financial risk, evaluation of financial responsibility matrices for improvement opportunities, and ensure overall timely renewal of existing payor contracts. Create and maintain a payor contract renewal calendar across all payors and lines of business. Assist internal areas with annual payor audit readiness. May be involved in other departmental projects and committees as necessary.
Functions as an AltaMed ambassador, is welcoming and attentive to the needs of all patients and creates a positive patient experience by managing expectations while demonstrating compassion and respect. The M.A. II will assist in the provision of direct patient care by performing routine patient care procedures and technical/clerical support services in the clinical setting. He/She will perform this role by building relationships with patients, listening to them and gaining their trust. He/She will also demonstrate compassion and respect and will be proactive in managing patient’s expectations. In addition, the M.A. II will document all specialties in the Electronic Health Record (EHR) and assist with more administrative responsibilities that include providing feedback on Back Office processes to the Nursing Supervisor/Manager. The RN Case Manager will provide daily care coordination with concurrent review for patient in the hospital or Skilled Nursing Facilities (SNF) in person or telephonically. This position will also provide case management to patients who are admitted to the hospital and those patients who may need to be enrolled in ambulatory case management. The RN case manager will be responsible for identifying (California Children Services) CCS cases, handle transfers, and retro reviews. Works as part of an interdisciplinary care team coordinating care and collaborating with social work, ambulatory case management, and Regional Medical Directors, Hospitalist, the hospital nursing personnel, and with the physician in the clinic. The RN Case Manager will be expected to perform assessments and use the appropriate guidelines to ensure the patient is receiving the appropriate level of care.
The Registered Nurse (RN) assumes the responsibility for application of nursing and clinical processes and delivery of quality patient care at AltaMed clinics under the supervision of Nursing Supervisor, Nursing Director or CNE and has accountability of the nursing process and the delivery of efficient patient care. RN supports patient evaluation, and carries out related nursing intervention, supports nursing patient evaluation, efficient system applications and recording of patient care/behavior. Works collaboratively with clinic interdisciplinary team members, medical staff, clinical operations, and with the Administrative/Operational Leadership teams to ensure achievement of patient, clinical or program goals as appropriate. Provides clinical care and demonstrated clinical competency for area of specialty patient populations. Application of and knowledge of regulatory requirements in delegation of tasks and in documentation. Assist in the development and implementation of quality, cost-effective, efficient systems and protocols for standardizing processes of patient care at clinics. Within Health Services, may work as the Triage Nurse in Urgent Care, Primary Care Nurse in the Nurse Clinic or the Charge Nurse on the floor at each clinic. Nursing job duties are to comply with the Nurse Practice Act, Title 22, Joint Commission (JC), CMS 42 CFR Part 460 regulatory requirements as appropriate. The plan for provision of nursing care to patients aligns with the AltaMed Code of Conduct, professional and educational standards, requirements as needed.
The Physician Reviewer of Utilization Management will provide routine review of authorization requests from all lines of business using respective national/state, health plan, nationally recognized guidelines where appropriate to provide guidance for processing of referrals to Medical Director staff and to UM staff. Physician Reviewers may approve authorizations per delegation responsibilities in AltaMed Authorization Matrix and where applicable per restrictions of their training and licensing. Physician Reviewers will process Denials of authorizations using standard denial language noted from above hierarchy of guidelines and post recommendations in the authorization platform to inform Medical Director workflow. Physician Reviewers may review and make recommendations on retro claims reviews for outpatient authorization, inpatient authorizations, and Emergency Room claims. Physician Reviewer may be involved in ad hoc projects and analysis of high cost utilization areas, unmanaged care, inappropriate utilization, inappropriate billing practices, and budgeting/finances reporting. May be responsible for identifying high risk population via provider/clinic referral, utilization management referral, disease registry reporting mechanisms and patient self-referral and will refer high risk patients to appropriate case management program per AltaMed Case Management Policy. With the approval of the Medical Director of Medical Management, Physician Reviewers may provide oversight, guidance, and training sessions to UM nurse reviewers and other UM staff where applicable. This position will provide case management to patients who are admitted to the hospital and those patients who may need to be enrolled in ambulatory case management. The Physician Reviewer may work as part of an interdisciplinary care team participating in the coordination of care with social work and mental health counseling, psycho-social support services, in-home support, legal services, skilled nursing, home health, etc. Effectively collaborates with the hospitalist, the hospital nursing personnel, with members of the interdisciplinary care team and with the physicians in the clinic.

The Director, Care Continuum position serves as the operational dyad partner to the Vice President, Associate Chief Value Officer on the strategic development and operational implementation of the care and services that extend beyond AltaMed’s traditional in-clinic primary care model. The Director will lead the planning and delivery of services, partnering with Medical Directors, Regional and Clinic Operational and Nursing Leadership, and cross-functional departments to ensure alignment, integration, and optimization of between traditional ambulatory care services with specialty services, referral management processes, utilization management services and supporting our patients as they transition from acute back to ambulatory clinical services. The Director has operational responsibility for driving design, optimization and improvements across all clinical, operational and administrative services that impact population health management extending beyond traditional ambulatory care services, including deploying standard operational processes, performance measures, priorities, and policies to achieve quality and population health outcomes. In this role, the Director will work collaboratively with key healthcare professionals to identify opportunities for improvement, trend analysis, education, and the development of appropriate action plans for problem resolution. The Director will also be responsible for developing utilization management goals and objectives, coordinating the integration between medical management delegated activities, IPA, and contracted Health Plans, and ensuring the development of care coordination across the continuum leads to outcomes supportive of industry standards and high quality care.
