Under the direction of the Director, Voice of the Customer, the Patient Experience Specialist is responsible for developing and deploying patient experience strategies and programs throughout the lines of AltaMed business. The Specialist is a patient experience subject matter expert who will partner with leadership and various stakeholders to assess needs, identify opportunities for improvement, and create programs designed to enhance the patient experience, and engage physicians, staff, and patients. Programs may include but are not limited to the whole person program, customer service training approach, provider rewards and recognition, and the patient advisory council. The position will also support process improvement and change management efforts around the various Patient Experience Department programs.

This program director in collaboration with the Associate Chief Medical Officer & Vice President of Senior Care and Complex Illness, is responsible for the successful development, growth and strategic integration of the senior and complex care programs within clinic operations and in alignment with AltaMed’s value-based primary care model. As an operational program director, this role collaborates with all stakeholders and cross-functional departments, including Specialty Services, Hospital Partners, Quality, Nursing and Health Services to optimize patient experience while maximizing access, cost-effectiveness and achievement of quality outcomes. Responsibilities and accountability includes but is not limited to, the development and execution of financial, quality, service goals, objectives, initiatives, and action plans to achieve outcomes by identifying and monitoring key service line targets and metrics. Additionally, the program director will lead or support the development and implementation of training, communication and performance management of the AltaMed Plus Senior and Complex Care, program staff to ensure all services are meeting or exceeding critical performance measures in health plan compliance, strategic goals and mission and vision of the corporation. Lastly, the Program Director will drive and participate in policy and community advocacy work with internal and external entities to advance legislative changes affecting senior and complex care and maintain and expand collaborative efforts with multiple community agencies providing these services.

This position supports patient evaluation, and carries out related nursing intervention, supports nursing patient evaluation, efficient system applications and recording of patient care/behavior.   Works collaboratively with clinic interdisciplinary team members, medical staff, clinical operations, and with the Administrative/Operational Leadership teams to ensure achievement of patient, clinical or program goals as appropriate. Provides clinical care and demonstrated clinical competency for area of specialty patient populations.  Application of and knowledge of regulatory requirements in delegation of tasks and in documentation.  Assist in the development and implementation of quality, cost-effective, efficient systems and protocols for standardizing processes of patient care at clinics. Within Health Services, may work as the Triage Nurse in Urgent Care, Primary Care Nurse in the Nurse Clinic or the Charge Nurse on the floor at each clinic.  Nursing job duties are to comply with the Nurse Practice Act, Title 22, Joint Commission (JC), CMS 42 CFR Part 460 regulatory requirements as appropriate.  The plan for provision of nursing care to patients aligns with the AltaMed Code of Conduct, professional and educational standards, requirements as needed.
