Under the supervision of the Associate Medical Director & Clinic Administrator, provide medical services to clients, ensuring appropriate and professional medical services.
Under the direct care supervision of a Registered Nurse or physician, the Regional Float LVN provides nursing care within licensed scope of practice.
As a regional float, assignment locations will vary depending on business need and may be within either Los Angeles or Orange County. Assignments will vary in length and can range from a few weeks to a few months depending on the reason for supplemental coverage.

Under general medical supervision, and within regulatory scope of practice, assists in the provision of direct patient care by arranging patients for radiological examinations. Produces radiographs (x-rays) to aid in the diagnosis of medical problems. Prevents patient from being exposed to unnecessary radiation. In addition, performs routine medical assisting procedures such as administering intramuscular, subcutaneous or intra-dermal injections and performing skin tests, venipunctures and skin punctures for blood specimens. As a q2Q2level II position also provides x-ray-related technical support and professional expertise to the units of purchasing and clinical operations
Responsible for transportation services; receiving incoming/outgoing pick-up calls and scheduling transportation services for the same day. Communicating with PACE centers to coordinate daily transportation for AltaMed patients and participants. Assist with other transportation vendors (e.g. Agape) by facilitating information and back-up service when/if possible. Duties include, but are not limited to, scheduling, routing, record keeping, and auditing to ensure compliance of AltaMed’s policies and procedures. Dispatcher is responsible to ensure all drivers are commuting back to base at the end of the day once the assignments are completed. Dispatcher will also ensure compliance of program deadlines and provide support to others as needed.
The VP, Fund Development position will manage the strategic plan and implementation oversight and management to drive revenue and increase donors from direct marketing, major, planned, corporate & foundation giving and distinguished, relationship events for AltaMed Foundation President. All fundraising must align to AltaMed Health Services Corporation Strategic priorities and mission, i.e., improve patient outcomes and reduce health disparities. This role requires collaboration with the AltaMed Foundation’s Board of Trustees, with guidance from the CAO, to ensure fundraising goals are met. The VP of Fund Development will provide direction to the Development department in order to drive growth in existing channels while also developing new funding relationships with the Foundation President, Foundation’s board of trustees, academic institutions, corporations, and private donors; as well as expand their fundraising portfolio. The fundraising portfolio is comprised of corporate, major, employee donation and private donors. Internally, the VP of Fund Development and Community Support will partner with the Government Relations and Civic Engagement, and AltaMed’s Grant’s department to coordinate and align fundraising efforts.

This position is responsible for all aspects of nursing care and the supervision of clinical support and nursing staff. The Supervisor is responsible for overseeing the day-to-day staff activities. Will interact, coordinate, and facilitate activities with all disciplines, departments, and appropriate regulatory agencies.
Under the general supervision of the Facilities Supervisor and/or Site Administration, this position is responsible for cleaning the facility in such a way to promote health and safety while reducing the spread of infection.
This position will provide support to the pharmacist-led chronic disease management program and assist with chronic disease management and medication therapy management. The coordinator care management will review registry data and other data sources to determine based on guidelines which patients are eligible for chronic disease management and will perform outreach to enroll appropriate patients in the program.
The Physical Therapist - PACE position provides services that help restore function, improve mobility, relieve pain and prevent or limit permanent physical
disabilities of patients suffering from injuries or disease as they restore, maintain and promote overall fitness and health. Responsible for evaluating individuals
with mobility dysfunction and provide a plan of care that is specific to the individual’s goals related to functional independence.
Corporate Setting:
This position is also responsible for providing support to the Rehabilitation Manager in the design, development, implementation, and monitoring of rehabilitation
programs to improve outcomes, processes, and participant experience. This position is also involved in the onboarding and training of new rehabilitation staff
members and will provide coverage at the PACE sites as needed.

This position is responsible for all aspects of the ‘back office’ including nursing care and the supervision of clinical support and nursing staff by overseeing the day-to-day clinical staff activities and ensuring the patient care staff provide high quality, accessible, cost effective care and patient focused services. Additionally, this position will implement and maintain clinical and patient care practice standards, budgets, and staff development.
Functions as an AltaMed ambassador, is welcoming and attentive to the needs of all patients and creates a positive patient experience by managing expectations while demonstrating compassion and respect. The M.A. II will assist in the provision of direct patient care by performing routine patient care procedures and technical/clerical support services in the clinical setting. He/She will perform this role by building relationships with patients, listening to them and gaining their trust. He/She will also demonstrate compassion and respect and will be proactive in managing patient’s expectations. In addition, the M.A. II will document all specialties in the Electronic Health Record (EHR) and assist with more administrative responsibilities that include providing feedback on Back Office processes to the Nursing Supervisor/Manager. Will also assist in achieving department goals and objectives by providing support and guidance to the M.A. team as a mentor and trainer. If working at the Patient Service Center, will provide other duties over the telephone.
The Program Director, Senior Care and Complex Care position serves as the dyad operational partner to the Associate Chief Medical Officer & Vice President of Senior Care and Complex Illness on the strategic development and operational implementation of care models aimed at improving care for aging and complex chronic disease patients. This program director in collaboration with the Associate Chief Medical Officer & Vice President of Senior Care and Complex Illness, is responsible for the successful development, growth and strategic integration of the senior and complex care programs within clinic operations and in alignment with AltaMed’s value-based primary care model. As an operational program director, this role collaborates with all stakeholders and cross-functional departments, including Specialty Services, Hospital Partners, Quality, Nursing and Health Services to optimize patient experience while maximizing access, cost-effectiveness and achievement of quality outcomes. Responsibilities and accountability includes but is not limited to, the development and execution of financial, quality, service goals, objectives, initiatives, and action plans to achieve outcomes by identifying and monitoring key service line targets and metrics. Additionally, the program director will lead or support the development and implementation of training, communication and performance management of the AltaMed Plus Senior and Complex Care, program staff to ensure all services are meeting or exceeding critical performance measures in health plan compliance, strategic goals and mission and vision of the corporation. Lastly, the Program Director will drive and participate in policy and community advocacy work with internal and external entities to advance legislative changes affecting senior and complex care and maintain and expand collaborative efforts with multiple community agencies providing these services.

The Site Medical Director has overall responsibility for the clinical patient care. He/she addresses clinical, programmatic, medical, and administrative issues at the site in collaboration with the Clinic Administrator and Nurse Administrator. The Site Medical Director or designee actively participates in the Interdisciplinary Team and directs the medical care decisions.
