Under the direction of Director of the Strategic Project Management Office, the Senior Project Manager will be responsible for managing large and/or complex programs and projects throughout AltaMed, including the clinical sites with the aims of improving patient health outcomes and business results. The focus will be to design, plan, manage and implement the highest priority strategic business initiatives with significant financial, process, information systems and organizational impacts that span across multiple business lines and functions. Key attributes include the ability to effectively communicate and interact with senior business leaders and other project and program sponsors, quickly and comprehensively assess and address issues associated with supported initiatives, and the ability to identify and implement improvements within the overall project delivery process. She/he will be expected support and coach Project Management (PMBOK) processes and best practice development and to contribute in the department’s effort to institutionalize project management, program management, change management and lean processes throughout the enterprise.

Under the supervision of the site Center Manager, the Care Center Supervisor is responsible for supporting all aspects of selected operational areas at the assigned site. These operations may include but not limited to clinical support, participant personal care, recreational therapies, administrative, nutritional, participant transportation and/or day center activities, all of which ensure the efficient and effective delivery of health care and social services at the AltaMed PACE center. He/She will be accountable to support the achievement of efficient operations in accordance with licensing, regulatory standards and company policies and procedures. Additionally, he/she will contribute administrative skills and abilities to the staff selection and retention process, annual budgets, process improvement efforts and care coordination with internal and external resources.
The CORE Coordinator II is a key central role within AltaMed’s Community Organizing and Research Engagement (CORE) team and reports directly to the CORE Manager. AltaMed’s CORE Team was established within the AltaMed Institute for Health Equity as an evidence-based immigrant centered community driven process framework to advance health equity and social justice, in which community residents and patients are engaged as partners in addressing social determinants of health and underlying inequities. The CORE Coordinator I is responsible for the synchronization of all operations involved in the successful completion of projects within the CORE Team. The Coordinator will support implementation of CORE Team programs and projects. Activities will include maintaining project plans, calendars, timelines, and dashboards; assisting with budgets; write summary reports for community members, program partners, senior leaders, and funders, and disseminate key outcomes/best practices. The Coordinator will also mentor Project Interns and Community Health Organizers to implement outreach and engagement activities aligned with strategic community priorities.
Under the supervision of the Associate Medical Director & Clinic Administrator, provide medical services to clients, ensuring appropriate and professional medical services.
The Home Care Coordinator is responsible for the support and implementation of home care services for participants. Provide support with the evaluation of a safe home environment through home visit assessments and coordination with home caregiver vendors and family caregivers, coordination of durable medical equipment, and nursing home care. Attends scheduled IDT and Coordination meetings. Responsible for acting as the liaison between the IDT and the home care providers, she or he should possess good leadership and communication skills. In addition, the Home Care Coordinator should be able to identify and understand participants’ medical and social needs in order to evaluate the home care needs of participants.

The Activities Coordinator is responsible for actively participating in all care coordination activities including Assessments and Care Planning. The Activities Coordinator is responsible for utilizing variety of techniques including but not limited to arts and crafts, animals, sports, games, dance and movement, drama, music, and community outings. The activity program shall also provide a planned schedule of social and other purposeful independent or group activities. This position is responsible for developing and implementing therapeutic activity program, to improve or maintain physical, cognitive, social, emotional and spiritual functioning in order to facilitate full participation in life. Activities Coordinator may use remote technology depending on the participants’ comfort level, electronic devices, and interests.

The Activities Coordinator is responsible for developing and implementing therapeutic activity programs. This position will also assist the Recreational Manager in the development and execution of recreational activities projects for process improvement and enhancement of participant experience.
The Health Services Operations Specialist supports the initiation, execution and monitoring of key initiatives aligned with Health Services' strategic priorities to achieve operational, quality and population health outcomes. This includes managing strategic initiatives across divisions’ business models and programs, creating structure, supporting the development and implementation of standard operating procedures and standardized work processes, and engaging clinical and operational workgroups and teams in deploying and adopting processes to achieve goals and outcomes. The Specialist is responsible for developing and maintaining detailed project plans, providing project administration support at the program level, and actively participating in financial validation, data analysis, and project controls. Additionally, the role involves collaborating with stakeholders to support the delivery of care and services to AltaMed's complex care population, as well as working with functional leaders, consulting partners, and initiative teams to drive change management activities and continuous improvement in business performance. The Specialist also plays a crucial role in managing initiatives aimed at aligning administrative service line processes across the enterprise. Overall, the Specialist is responsible for supporting the successful implementation of programs or projects that align with strategic goals by utilizing effective project management tools and best practices.
The Physician Reviewer of Utilization Management will provide routine review of authorization requests from all lines of business using respective national/state, health plan, nationally recognized guidelines where appropriate to provide guidance for processing of referrals to Medical Director staff and to UM staff. Physician Reviewers may approve authorizations per delegation responsibilities in AltaMed Authorization Matrix and where applicable per restrictions of their training and licensing. Physician Reviewers will process Denials of authorizations using standard denial language noted from above hierarchy of guidelines and post recommendations in the authorization platform to inform Medical Director workflow. Physician Reviewers may review and make recommendations on retro claims reviews for outpatient authorization, inpatient authorizations, and Emergency Room claims. Physician Reviewer may be involved in ad hoc projects and analysis of high cost utilization areas, unmanaged care, inappropriate utilization, inappropriate billing practices, and budgeting/finances reporting. May be responsible for identifying high risk population via provider/clinic referral, utilization management referral, disease registry reporting mechanisms and patient self-referral and will refer high risk patients to appropriate case management program per AltaMed Case Management Policy. With the approval of the Medical Director of Medical Management, Physician Reviewers may provide oversight, guidance, and training sessions to UM nurse reviewers and other UM staff where applicable. This position will provide case management to patients who are admitted to the hospital and those patients who may need to be enrolled in ambulatory case management. The Physician Reviewer may work as part of an interdisciplinary care team participating in the coordination of care with social work and mental health counseling, psycho-social support services, in-home support, legal services, skilled nursing, home health, etc. Effectively collaborates with the hospitalist, the hospital nursing personnel, with members of the interdisciplinary care team and with the physicians in the clinic.
The Coordinator, Scheduling position is responsible for scheduling and coordinating off-site specialist appointments for urgent, routine and follow-up appointments in accordance with the approved authorizations, scheduling and turnaround time objectives. This position works closely with members of the IDT team to coordinate services and advocate for participants specialty needs. The Scheduling Coordinator advocates for participant’s specialty needs, and is an integral part of the care team, working as a liaison between the participant, primary care physician (PCP), specialty providers and Utilization Management to ensure timely access to specialty care.
Under the direction and oversight of Director of Project Management, this person will be responsible for supporting strategic initiatives within Health Services and project management teams by defining project parameters and developing project documentation, including business requirements, project plans, project schedules, workflow solutions, and project status reports/communications. He/She will interact with project teams and be responsible for conducting activities that support project execution including financial validation, data analysis, developing of recommendations and action plans, and assist with research, analysis, and preparation of data and studies. Additionally the position will provide project administrative support at the portfolio level; review change orders, document lessons learned, update documentation and schedules as required for each project and phase, schedule and conduct project meetings to include logistics, agendas, and meeting minutes. May be assigned to lead key work components within a larger project.

The Pharmacist In Charge – Infusion position is a licensed professional who is responsible for providing safe and appropriate pharmacy services to AltaMed patients in accordance with the policies and procedures of the AltaMed, pharmacy practice professional standards, and applicable regulatory entities. This is an entry level position into the practice of clinical pharmacy in the alternate setting. The staff Clinical Pharmacist coordinates patient services with the Nursing Department, monitors patient progress toward goals and educates patients, caregivers, staff and other professionals regarding appropriate medication use.
Works with a Radiologist to perform diagnostic imaging examinations that are crucial for the detection of illness, injury, and other medical conditions. The
Certified Radiology Technician produces high-quality radiographs (x-rays) to aid in the diagnosis and treatment of medical problems. Provides essential support
to ensure patients are comfortable and at ease throughout the course of the procedure. Prevents patients from being exposed to unnecessary radiation.
