Provide a psychosocial perspective to the interdisciplinary evaluation, assessment, plan of care, ongoing services, and disenrollment processes of the PACE program. Interventions may include individual participant and/or family contacts, collateral contacts, participant and family education, assessment, and counseling; mobilization of resources; identification and management of behavioral health needs; case management and advocacy; and discharge planning. Use knowledge of social systems and individual behavior to skillfully apply interventions that meet the unique needs of PACE participants and their families. The MSW collaborates as part of the Interdisciplinary Team in efforts to optimize health status and quality of life of the PACE participants.

The Director of Accreditation and Regulatory Affairs is responsible for leading and directing the attainment and maintenance of the Joint Commission (JC) accreditation, ensuring survey readiness. The position coordinates verbal and written communication with the JC and other regulatory agencies. Collaborates with AVP’s, Clinic Leadership, Clinic Staff, Site Medical Directors and Clinic physicians to assist with and coordinate all regulatory functions. Works with clinics/teams to develop quality action plans for improvement.

The M.A. II will assist in the provision of direct patient care by performing routine patient care procedures and technical/clerical support services in the clinical setting. He/She will perform this role by building relationships with patients, listening to them and gaining their trust. He/She will also demonstrate compassion and respect and will be proactive in managing patient’s expectations. In addition, the M.A. II will document all specialties in the Electronic Health Record (EHR) and assist with more administrative responsibilities that include providing feedback on Back Office processes to the Nursing Supervisor/Manager. Will also assist in achieving department goals and objectives by providing support and guidance to the M.A. team as a mentor and trainer.

Under the supervision of the PACE Regional Director of Senior Care Services, responsible for the overall operations of the SBC PACE center. Works with key management personnel, in achieving services according to licensing standards and/or regulatory agency standards, which are applicable to PACE.

The Nurse Case Manager may provide daily care coordination, case management, coaching, consultation and intervention to patients with one or more chronic diseases. May also be responsible for identifying said population via provider/clinic referral, utilization management referral, disease registry reporting mechanisms and patient self-referral. This position may also provide case management to patients who are admitted to the hospital and those patients who may need to be enrolled in ambulatory case management. The case manager will be responsible for identifying (California Children Services) CCS cases, handle transfers, and retro reviews. Works as part of an interdisciplinary care team coordinating social work and mental health counseling, psycho-social support services, in-home support, legal services, skilled nursing, home health, etc. The Nurse Case Manager effectively collaborates with the hospitalist, the hospital nursing personnel, with members of the interdisciplinary care team and with the physician in the clinic.

Under the direct care supervision of a Registered Nurse or physician, the Regional Float LVN provides nursing care within licensed scope of practice.
As a regional float, assignment locations will vary depending on business need and may be within either Los Angeles or Orange County. Assignments will vary in length and can range from a few weeks to a few months depending on the reason for supplemental coverage.

Provides administrative support to the Regional Floats and to the LVN supervisor. Responsibilities may include: coordination of regional float clinic assignments and the maintenance of staffing requisitions. This individual is also responsible for managing regional floats overtime and payroll. Other duties include communicating assignment expectations to the regional floats, composing memos, transcribing notes, handling multiple projects, and preparing and monitoring invoices and expense reports.
