This position is under the guidance and direct supervision of the PACE Supervisor. The Personal Care Attendant I provides support to participants across all platforms of delivery of care. Ability to assist in the coordination, preparation of all food services, support and assist center activities, coordinate the delivery of quality activity program/activities for all participants. Ensure that the recreational, physical, intellectual, spiritual, and ensures social needs of each patient is met. Accompany/assist PACE participants to and from outside specialty appointments.
The Change Management and Training Advisor (CMTA) will be responsible for leading the creation and execution of Prosci based change management strategies that maximize adoption of strategic initiatives, business processes, organizational changes, systems and technology. This position will focus on the people side of change and identify, oversee, and ensure the successful execution of all change management activities for identified projects to drive adoption and lasting change. The CMTA shall have familiarity with project management approaches, tools and phases of the project lifecycle and will work closely with project managers and project teams in integrating change management activities into their project plans. The CMTA will have to work through many others in the organization to succeed and shall provide direct support to various stakeholders and coaching to all levels of managers and supervisors as they help their direct reports through transitions. These improvements will increase benefit realization, value creation, ROI and the achievement of results and outcomes.
Provides administrative support to the assigned unit. Responsibilities may include: scheduling appointments, giving information to callers, dictation, composing memos, transcribing notes, handling multiple projects, and preparing and monitoring invoices and expense reports. May also research and create presentations, generate reports and represent management at meetings as needed.

The Director of Accreditation and Regulatory Affairs is responsible for leading and directing the attainment and maintenance of the Joint Commission (JC) accreditation, ensuring survey readiness. The position coordinates verbal and written communication with the JC and other regulatory agencies. Collaborates with AVP’s, Clinic Leadership, Clinic Staff, Site Medical Directors and Clinic physicians to assist with and coordinate all regulatory functions. Works with clinics/teams to develop quality action plans for improvement.
This position is responsible for the review of medical records related to patient safety and potential quality of care concerns specific to patient complaints, grievances and appeals. This position is also responsible for putting together responses to health plans and regulatory agencies inquiries and developing corrective action plans in collaboration with other internal departments. The Nurse will participate in health plan and regulatory audits to ensure compliance, sustainability, and improve patient safety and quality assurance.
Under the direct supervision of the PACE Supervisor. The PCA II will perform and assist with personal care ADLs/IADL’s for participants in the PACE across all platforms of delivery of care like center and/or in the home or virtual. The PCA II must be able to perform the job duties of a PCA I if assigned. In addition, the PCA II supports the PCA III in the gathering and reporting information related to the care coordination, participant’s ability to engage, maintain/ improve their ability, foster independence, maintain function or improve socialization, or satisfaction. The PCA II communicates and collaborates with IDT members or other PACE staff in the goals of the plan of care, focusing on needs (like participant supplies or tools), and health status, physical and mental well-being.
Responsible for the development and implementation of homecare services for program participants, including the coordination of all contracted home services, durable medical equipment and nursing home care. Attends scheduled IDT and Coordination meetings.

The M.A. II will assist in the provision of direct patient care by performing routine patient care procedures and technical/clerical support services in the clinical setting. He/She will perform this role by building relationships with patients, listening to them and gaining their trust. He/She will also demonstrate compassion and respect and will be proactive in managing patient’s expectations. In addition, the M.A. II will document all specialties in the Electronic Health Record (EHR) and assist with more administrative responsibilities that include providing feedback on Back Office processes to the Nursing Supervisor/Manager. Will also assist in achieving department goals and objectives by providing support and guidance to the M.A. team as a mentor and trainer. If working at the Patient Service Center, will provide other duties over the telephone.

The Encounter Data Specialist is responsible for managing all aspects of encounter data through all contracted health plans that supports AltaMed’s managed care initiatives. This position manages encounter data submissions through methodical investigation, analysis, review and documentation of multiple levels of requirements from an end-to-end perspective and supports the ongoing management of the requirements. This position will assist in report creation and audit/reconcile health plan encounter data submissions. It is responsible in communicating and coordinating with IT, PFS, clearinghouses and contracted health plans on encounter related efforts. Encounter Data Specialist will also coordinate and manage the implementation and/or integration of products and support of the encounter inbound and outbound functions. The Encounter Data Specialist will maintain knowledge of all HIPAA EDI protocols, X12 transaction sets and other EDI processing standards.
Community Health Specialist (CHS) have the skills and experience to understand what at-risk or vulnerable patients are going through and help them get through difficult times. CHSs are people who come from the communities they serve. CHSs act as caring neighbors to help patients address the social, medical and community problems that lead to poor health. The goal is to help patients address real-life obstacles (e.g. social determinants of health) that keep them from staying healthy, help with the tasks of getting medical care, and work on health goals by doing things like planning healthy meals or finding time to exercise.
Under the oversight of the CHW Program Manager, and by following an evidence based program and structure, the Community Health Specialist is responsible for providing patients with support to address the challenges that prevent them from fully engaging in their health care.
Under the direction of the IT Support Supervisor and/or IT Support Lead Technician, performs a variety of technical support services related to the installation, operation, repair and diagnosis of computer equipment and related services. Also provides technical user support assistance both remote and at the desk side.

A Claims Examiner is responsible for analyzing and the adjudication of medical claims as it relates to managed care. Performs payment reconciliations and/or adjustments related to retroactive contract rate and fee schedule changes. Resolve claims payment issues as presented through Provider Dispute Resolution (PDR) process or from claims incident/inquiries. Identifies root causes of claims payment errors and reports to Management. Responds to provider inquiries/calls related to claims payments. Generates and develop reports which include but not limited to root causes of PDRs and Incidents. Collaborates with other departments and/or providers in successful resolution of claims related issues.

The LVN Infection Prevention Specialist helps to prevent healthcare-acquired infections (HAIs) by implementing protocols to isolate sources of infections and limiting their spread; systematically collects data, and applies the prescribed Infection Control plan. In collaboration with the Director of Infection Prevention, monitor appropriate public health practices; and trains healthcare staff through instruction of information on infection control practices. This individual is responsible for assisting and supporting the implementation of the Regulatory requirements as stipulated by: CMS, DHCS, HRSA, OSHA, CDC, The Joint Commission, and California Department of Public Health, to ensure that Sterilization and High Level Disinfection processes and Infection Prevention practices meet regulatory standards.
The Sterilization Coordinator will work in partnership with the LVN Infection Prevention Specialist, Nurse Supervisors, Dental Services Specialists, as well as the individual Autoclave Specialists and employees at the clinic and dental sites. The Sterilization Coordinator will implement Sterilization and High Level Disinfection Programs, while providing onsite support. The Sterilization Coordinator will provide direct observation of the infection prevention precautions, as well as the Sterilization and High Level Disinfection practices that occur in the clinic/dental environment. This individual will share all concerns and new developments as they occur with the Director of Infection Prevention. Sterilization Coordinator will ensure that the Nurse Supervisors, the Autoclave Specialists and employees are trained, competent, and suited for their roles and in compliance with Federal, State and OSHA regulations.
This individual will help to enhance a culture of safe sterilization and disinfection processes, by assisting the Director of Infection Prevention to increase awareness of infection prevention practices, ensure engagement of the Autoclave Specialists by providing education, monitoring to ensure adherence to the program guidelines. This individual will assist with the prevention of infectious diseases by observing activities in the employee work environment. The Sterilization Coordinator, supports the Director of Infection Prevention in the development and execution of continuous process improvement initiatives, while collaborating with cross-functional leaders in Health Services, Ancillary Services, Senior Care Services and Dental Services.
